4: Death Certificate

• You will be redirected to www.LoomisFamilyCremations.com/product/death-certificate
• Make desired selections
• Click “Next” button

5: Checkout

• You will be redirected to www.LoomisFamilyCremations.com/checkout
• Complete Billing Address fields
• Complete Additional Information fields (who the arrangement is for)
• Enter credit card information
• Click “Place Order” button

6: Order Received

• You will be redirected to www.LoomisFamilyCremations.com/checkout/order-received
• Click “Continue” button

7: Online Arrangement Form

• You will be redirected to www.LoomisFamilyCremations.com/online-arrangement-form/
• Complete all fields
• Click “Submit” button to continue

 

8: Cremation Authorization Form

• You will be redirected to www.LoomisFamilyCremations.com/online-secure-forms/
• Follow the instructions for completing the Cremation Authorization Form.
• After completing the Cremation Authorization Form in SignNow, complete the Acknowledgement Form on this page.
• Click “Submit” button to continue
